User-supplied information: If you fill out the contact form on this website and/or submit payment through the RevExpress or Fifth Third Biller Direct portals, we will ask you to provide personal information, such as e-mail address, name, home & cell phone numbers, etc.  Your submission of this personal information shall also serve as consent for MMCB and its contractors to use this information to contact you at any point in the future.  Please do not submit any confidential, proprietary or sensitive personally identifiable information (e.g. Social Security Number; date of birth; drivers license number – collectively, “Sensitive Information”).  If you submit any Sensitive Information, you are doing so at your own risk and MMCB will not be liable to you or responsible for any resulting consequences of your submission.   Any correspondence will be made in attempt to collect a debt and any information obtained will be used for that purpose.

Web server logs: When you visit our website, we may track information about your visit and store that information in web server logs, which are records of the activities on our sites.  Servers automatically capture and save the information electronically.  Examples of the information we may collect include:
